Prescribing Opioids and Benzodiazepines Strategies for Harm Reduction and Monitoring

At the conclusion of this course, participants will be able to:

This activity provides comprehensive, evidence-based training for clinicians managing chronic pain and opioid therapy, aligning with the 2022 CDC Clinical Practice Guidelines and a whole-person, biopsychosocial approach to patient care. The course moves beyond basic compliance by detailing the practical application of universal precautions, including a nuanced interpretation of urine drug testing (UDT) for fentanyl and xylazine, and effective use of Prescription Drug Monitoring Programs (PDMP) to prevent misuse. Key training focuses on the “how-to” of responsible prescribing: calculating safe morphine milligram equivalents (MME), implementing slow, patient-centered tapering protocols to avoid destabilization, and recognizing the physiological signs of opioid-induced hyperalgesia (OIH). The course also emphasizes opioid-sparing strategies, prioritizing multimodal analgesia and nonpharmacologic interventions as primary tools to support functional restoration and long-term safety.

The educational need for this course is driven by the emergence of the "Fourth Wave" of the opioid epidemic, characterized by the saturation of the drug supply with illicit fentanyl and xylazine, and the concurrent rise in overdose mortality among older adults and minority populations.
