Safer Transfusions: Recognition, Response, and Reporting

At the conclusion of this course, participants will be able to:

1. Identify core principles of blood product stewardship during shortages, including ethical allocation and restrictive transfusion strategies.

2. Recognize clinical features of TACO, TRALI, and acute hemolytic transfusion reactions using case-based cues and diagnostic considerations.

3. Describe immediate response steps and documentation/reporting actions that support hemovigilance, patient safety, and quality improvement.

This course focuses on safe transfusion practice during routine care and periods of blood product scarcity. It covers core principles of blood product stewardship, practical clinical triage for appropriate use, and recognition and response to serious transfusion reactions including TACO TRALI and acute hemolytic transfusion reactions. Learners also review the essential documentation steps and reporting pathways used in hemovigilance programs to reduce preventable harm and support quality improvement.

This activity addresses common practice gaps in transfusion stewardship, delayed recognition of life-threatening transfusion reactions, and inconsistent documentation and reporting that can undermine patient safety and institutional learning.
