End-of-Life Care: Integrating Pain Management, Communication, and Ethical-Legal Decision Making

At the conclusion of this course, participants will be able to:

1. Analyze the role of interdisciplinary care teams in optimizing patient comfort and supporting families during the terminal phase of illness.

2. Formulate an individualized, evidence-based pain and symptom management plan for terminally ill patients, including dose adjustments and routes of administration for opioids and adjuvant medications.

3. Describe effective communication strategies for discussing prognosis, care goals, and navigating difficult conversations about withholding or withdrawing life-sustaining treatments with patients and families.

4. Justify clinical decisions in end-of-life care by integrating ethical principles, relevant legal statutes (e.g., advance directives), and the whole-person approach that integrates physical symptoms with psychological, social, and spiritual needs.

This activity provides a comprehensive, evidence-based review of end-of-life (EOL) and palliative care for healthcare professionals involved in the care of patients with serious or life-limiting illnesses. Participants will be guided through the critical clinical, ethical, and communication skills necessary for managing patients during the terminal phase of illness. The course begins by establishing a foundational understanding of the difference between palliative and hospice care, and the holistic mandate of the Interdisciplinary Team (IDT). It emphasizes evidence-based pharmacological formulation for complex symptoms (pain, dyspnea, delirium), introduces structured communication models (SPIKES, SICG) for prognosis discussions, and culminates in a detailed exploration of the ethical and legal justification for withholding or withdrawing life-sustaining treatment, reinforced by the core principles of whole-person, patient-centered care.

The educational need for this course arises from recurring state requirements and the significant gap in clinician preparedness for managing the complex, non-physical dimensions of death and dying. Clinicians frequently lack the structured training necessary to approach conversations about prognosis, manage family conflict over treatment limitations, and confidently apply ethical principles (autonomy, proportionality) to LST decisions. This activity closes that gap by providing practical, evidence-based protocols in symptom management, detailed communication strategies to mitigate family distress, and a structured legal/ethical framework to justify care decisions that prioritize patient dignity and comfort over futile intervention.
