Ohio Pharmacy Standard Renewal Requirements: Pharmacists in Ohio must complete 30 hours (3 CEUs) of continuing education every two years, with mandatory 2 hours each in medication‑errors/patient‑safety and law/jurisprudence , and no required live hours — all formats are accepted and must fall within the two‑year period preceding September 15 of the renewal year. Up to 10 of the 30 hours can come from approved volunteer healthcare services, and board‑approved specialty certification may substitute most CE requirements, aside from the obligatory law and safety hours. I was hospitalized last year for almost a month and I noticed multiple medical errors (not being hooked up to my IV multiple times, given wrong medication which caused my sodium to be very low, etc.) which lengthened my stay. I can now define those exact errors and better understand what happened to me. I hate that these medical errors are so common and I appreciate how well laid out your breakdown and categorization of them was.
