South Carolina Pharmacy Standard Renewal Requirements: Pharmacists in South Carolina must renew their licenses annually by March 31 and complete 15 hours of ACPE or CME Category 1 continuing education each license year. 7.5 hours must focus on drug therapy or patient management, with mandatory CE including 1 hour on controlled substances , 1 hour on immunizations (if administering) , and 1 hour on hormonal contraceptives (if under the Access Protocol). I was hospitalized last year for almost a month and I noticed multiple medical errors (not being hooked up to my IV multiple times, given wrong medication which caused my sodium to be very low, etc.) which lengthened my stay. I can now define those exact errors and better understand what happened to me. I hate that these medical errors are so common and I appreciate how well laid out your breakdown and categorization of them was.
