Washington Pharmacy Standard Renewal Requirements: Washington pharmacists must renew their licenses every two years—on their birthday—and fulfill 30 hours of continuing education, all of which may be completed via home study and must be delivered by ACPE‑accredited providers, except for the mandated suicide prevention and health equity trainings. CE must be tracked through the state's designated system and cannot roll over between cycles. Washington Department of Health

I was hospitalized last year for almost a month and I noticed multiple medical errors (not being hooked up to my IV multiple times, given wrong medication which caused my sodium to be very low, etc.) which lengthened my stay. I can now define those exact errors and better understand what happened to me. I hate that these medical errors are so common and I appreciate how well laid out your breakdown and categorization of them was.
