Additional information

Vermont Pharmacy Standard Renewal Requirements: Pharmacists in Vermont must renew their licenses biennially by July 31 of odd‑numbered years, completing 30 hours of CE per cycle. At least 10 hours must be live , while the remaining hours can be completed via home study. Additionally, pharmacists authorized to immunize or handle controlled substances must fulfill 2 hours of topic‑specific CE each cycle. No CE credit carries over, and the Board may grant hardship waivers where applicable. Vermont Board of Pharmacy

---

First‑time renewal: For newly licensed pharmacists, CE accumulation begins with the first full biennial renewal period following licensure. Those licensed within 90 days of a renewal deadline start CE tracking at the beginning of that cycle for the full two-year period. Vermont Board of Pharmacy

---

Immunization practice & CPR: Pharmacists authorized to administer vaccines must complete 2 hours of immunization-related CE each cycle. Vermont Board of Pharmacy

---

Controlled substance or opioid CE: Pharmacists prescribing or dispensing controlled substances must complete 2 hours of CE on topics related to controlled substances (e.g., abuse, diversion, safe use, storage, disposal, non‑opioid alternatives, tapering, legal aspects) each renewal cycle. Vermont Board of Pharmacy

I really loved this information. I was hospitalized last year for almost a month and I noticed multiple medical errors (not being hooked up to my IV multiple times, given wrong medication which caused my sodium to be very low, etc.) which lengthened my stay. I can now define those exact errors and better understand what happened to me. I hate that these medical errors are so common and I appreciate how well laid out your breakdown and categorization of them was.
