Preventing Medical Errors CME for Physicians - 2 AMA PRA Category 1 Credits™

Upon completion of this activity, participants will be able to:

1. Differentiate common diagnostic inaccuracies across oncology-related, neurology-related, cardiology-related, infectious disease-related, and gastroenterology-related presentations, and apply structured diagnostic reasoning to reduce missed and delayed diagnoses.

2. Analyze system failures and human factors that contribute to diagnostic, medication, and procedural errors by applying Root Cause Analysis to clinical scenarios.

3. Implement evidence-based error prevention strategies such as pre-procedural screening, informed consent, medication reconciliation, and standardized handoff communication.

4. Formulate system-level safety recommendations, including standardized protocols, EHR-based decision support, structured reporting pathways, and nonpunitive reporting cultures, that physicians and physician assistants can support or advocate for within their organizations.

This activity prepares physicians and physician assistants across specialties to recognize, prevent, and respond to medical errors in everyday practice. Participants will learn to identify high-frequency diagnostic failure patterns, including missed oncology, neurology, cardiology, infectious disease, and gastroenterology diagnoses. They will learn to apply structured diagnostic reasoning to reduce error. The course also provides working frameworks for Root Cause Analysis, Just Culture, error reporting and disclosure, and system-level safety design. These tools equip clinicians to improve patient safety in their own practice and within their healthcare organizations.

Florida Board of Medicine Relicensure

This activity is designed to satisfy the Florida Board of Medicine’s biennial requirement for two hours of continuing medical education on the Prevention of Medical Errors, including review of the five most misdiagnosed conditions, in accordance with Rule 64B8-13.005, F.A.C. The course is also suitable as general AMA PRA Category 1 Credit™ for physicians in other jurisdictions.

Educational Need

Most medical errors come from system failures rather than individual recklessness, and most are preventable. Diagnostic error alone accounts for a significant share of preventable harm, and medication and procedural errors remain persistent challenges across settings. Physicians and physician assistants often know that safety matters but lack concrete strategies to change practice. This activity closes that gap by teaching clinicians to recognize cognitive biases, apply pre-procedural risk screening, communicate well during handoffs and transitions, engage patients and families as partners, and advocate for system improvements. The principles apply universally, and examples are drawn from a range of specialties and practice settings.
